What is a ACH debit authorization form?

An ACH debit authorization form is a signed document that gives an organization permission to electronically withdraw funds from your bank account through the Automated Clearing House network. It identifies you as the account holder, specifies your bank routing and account numbers, and defines the terms of the withdrawal — whether it is a one-time debit or a recurring payment.

Landlords, gyms, insurance companies, utility providers, and subscription services commonly require this form before they can set up automatic payments from your account. The form serves as written proof of your consent, protecting both you and the organization initiating the debit.

Some organizations also request a voided check alongside this document. Generate a voided check →

Frequently asked questions

What are the common mistakes to avoid when filling out this form?
  • Authorizing debits from the wrong account typeSome organizations only accept ACH debits from checking accounts, not savings. Confirm with the organization before authorizing debits from a savings or money market account.
  • Forgetting to track authorized debitsOnce you authorize automatic withdrawals, it’s easy to lose track of them. Keep a record of every ACH authorization so you know exactly who has permission to debit your account.
  • Not reading the payment frequency carefullyMake sure you understand whether you’re authorizing a one-time debit or recurring withdrawals. A recurring authorization stays in effect until you revoke it in writing.
  • Giving authorization to an unverified organizationAn ACH authorization gives someone permission to pull money from your account. Only provide it to organizations you trust and have verified. If in doubt, pay by another method.
What is an ACH debit authorization form?

An ACH debit authorization form is a written document that gives an organization permission to electronically withdraw funds from your bank account. It specifies your bank details, the payment amount and frequency, and includes your signed consent. The organization uses this form to initiate ACH debits through the banking system.

How do I cancel an ACH debit authorization?

You can revoke an ACH authorization by notifying the organization in writing. You should also contact your bank to place a stop payment on future ACH debits from that organization. Federal law requires that your revocation be honored once the organization receives your written notice.

Is an ACH debit the same as a direct deposit?

No. A direct deposit is money coming into your account (a credit). An ACH debit is money going out of your account (a withdrawal). Both use the ACH network, but they move in opposite directions. This form authorizes debits — money being pulled from your account.

What bank information is needed for ACH?

The form requires your bank’s routing number (9 digits), your account number, and your account type (checking or savings). The routing number identifies the bank, and the account number identifies your specific account within that bank.

Is my banking information safe on this form?

Your bank details are used only to generate the form and are not stored permanently. However, you should only provide the completed form to organizations you trust, as it contains sensitive account information that could be used to initiate withdrawals.

Can I authorize both one-time and recurring payments?

Yes. The form includes a payment frequency field where you can specify one-time, weekly, bi-weekly, monthly, or other schedules. For recurring payments, the authorization remains in effect until you revoke it in writing.
